Insurance & nutrition care
Let’s make checking your coverage simpler.
We work with many major health plans. Your exact benefits depend on your employer, diagnosis and plan, but you can start with your insurance card and the steps below.
1. Find your plan
Browse the plan-type groups below. Older names are included when patients may still see them on a card.
2. Check while booking
Use the booking link to review current insurance options alongside available appointment times.
3. Confirm your benefits
Call the member-services number on your card and ask about outpatient medical nutrition therapy.
Plans and network names
Find the type of plan on your card
Scott Keatley and Gina Keatley have participation records with a broad range of plans and networks. The most commonly searched insurance families are shown first; open a plan-type group for the full list. Participation does not guarantee that your individual policy covers nutrition counseling.
Coverage is often used for condition-specific care, including diabetes and prediabetes nutrition counseling, heart-health nutrition counseling, and weight and metabolic health counseling.

Why show older names? Insurance companies merge and rebrand, but patients often search using the name printed on an older card or benefits document. A legacy name helps identify the plan history; it does not mean the former insurer is still selling coverage. Do not see your plan? Use “Check my insurance” above to review current options.

Good questions to ask
Insurance FAQ
Does being listed here guarantee that my visit will be covered?
No. Network participation and benefits are different. Your health plan—not this website—determines coverage, payment level and whether a referral or diagnosis is required.
What should I ask my insurance company?
Ask whether your plan covers codes 97802, 97803 and 97804, whether your diagnosis qualifies, how many visits are allowed, and what you may owe through a copay or deductible.
Do I need a referral?
Some plans require one and others do not. Download our referral form above if your plan or clinician requests it. Clinicians can also review our nutrition referral guidance.
Where can I find official Medicare information?
Visit the official Medicare website for current federal coverage and benefit information.
Ready when you are
Check your insurance while you choose an appointment time.
24-hour cancellation policy: Please notify the Keatley Nutrition team at 1-800-571-8276 at least 24 hours before your appointment to avoid the full missed-appointment charge. Monday appointments require Friday notice. Cancellation fees are charged to the client, not the insurance company.
Keatley Nutrition works to keep this page accurate, but networks and benefits can change. Please rely on your plan documents and insurer for final coverage decisions.
